Position Summary:

Under a grant from the Office of Refugee Resettlement (ORR) NYAPs Post Release Case Manager (Level I) provides Post Release (PRS) Level 1 services including checking the safety and well-being of children post release to sponsor families coordinating referrals to community-based services assessing child and sponsor family needs and elevating to a higher level of Post Release Services (PRS) if necessary. PRS Case Managers provide and foster a coordinated case management approach that adopts a collaborative process of assessment planning facilitation care coordination evaluation and advocacy for options and services to meet the unaccompanied child and the sponsors presenting needs.

Responsibilities:

  • Reports into PRS Caseworker Supervisor
  • Provide Level One (1) Post Release Services (PRS).
  • Conduct three virtual check-ins with post released children to confirm the child is residing with their sponsor enrolled in school aware of upcoming court dates healthy and safe.
  • Refer child for community-based services and assess needs; elevate to a higher level of PRS if necessary.
  • Carry a caseload not to exceed 40 cases per worker.
  • Ensure excellence in PRS documentation and Program compliance.
  • Establish relationships with community-based organizations.
  • Have a case management approach that assesses plans facilitates and advocates for unaccompanied children and their sponsors using knowledge of social services and human services.
  • Ensure proper reporting and elevation to ORR of timely notifications of concern and reporting to other stakeholders per state and ORR guidelines.
  • Provide and ensure effective communication with community partners community-based organizations and ORR stakeholders.
  • Demonstrate working knowledge and understanding of ORR Policy and Procedures Flores Agreement as well as other applicable and pertinent regulations and laws including the Trafficking Victims Protection Reauthorization Act (TVPRA) of 2008 .
  • Elevates any issues or concerns to NYAP leadership and ORR.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS

  • Must be at least 21 years old.
  • Bilingual in English and Spanish.
  • At least 1 year of experience working with children and youth.
  • Prior experience with ORR HS/PRS preferred.
  • Proficient in the use of desktop and laptop computers and ability to use the internet and related computer programs including ORRs Official System of Record (also known as the UC Portal).
  • Pass a criminal history screen including state and local child protection agency registries and C/ANs check.
  • Willingness to work flexible and non-traditional hours in the service of persons served.
  • Based on Program needs employee may be required to reside in region assigned.
