The Re-Entry Specialist provides case management and support services to help adults with recovery and re-entry needs transition successfully into the community. This role works closely with clinical teams peer coaches and members to develop individualized plans connect to resources and promote independence employment and community engagement.
Key Responsibilities:
Develop and implement individualized service/treatment plans.
Coordinate care resources and transitional services.
Support members with employment housing and community integration.
Maintain accurate case management documentation.
Advocate for members regarding insurance financial and service needs.
Collaborate with cross-functional treatment teams.
Provide transportation and assist with drug testing as needed.
Qualifications:
21 years old.
High school diploma/GED required; higher education or behavioral health experience preferred.
Peer & Recovery Support Specialist (PRSS) Certification required.
CPR/First Aid & Crisis De-escalation certification required.
Behavioral Health Technician (BHT) qualifications a plus.
Strong communication organizational and case management skills.
Valid AZ drivers license and ability to pass background drug TB and MVR checks.
Work Environment:
Outpatient clinical setting with some community-based activities.
Requires ability to drive document accurately and support members effective
