UPMC Community Medicine Inc. is seeking a part-time Internal Medicine or General Medicine Physician with experience in Palliative or Hospice Care to join UPMC Your Care located in Pittsburgh Pennsylvania.
About the Opportunity:
The Physician works independently and in collaboration with the Senior Medical to provide care to UPMC Health Plan members living with advanced chronic illness or potentially life-limiting conditions.
The physician assists with the diagnosis treatment and management of acute and chronic conditions by conducting a comprehensive assessment and development of patient centered plan of care including assessment and documentation of Hierarchical Condition Category (HCC) diagnoses as appropriate.
The physician is an integral part of the clinical management team and works collaboratively with the interdisciplinary team to manage the clinical and medical needs of the patients.
The physician communicates proactively with CRNPs and other care team members regarding the clinical condition of their patients.
The position will require travel for in-home visits. Must have a valid drivers license in good standing and reliable transportation.
The successful candidate will be placed according to years and months of experience for employment purposes.
Qualifications:
Demonstrate the knowledge and skills necessary to provide care and/or interact appropriately with the patients served as specified above
Demonstrate knowledge of the principles of growth and development over the life span
Possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to their age-specific needs
Provide the care needs as described in the department policy and procedures

Ability to establish and maintain positive caring relationships with executives managers physicians non-physician providers ancillary and support staff other departments and patients/families

Physical stamina for frequent walking standing lifting and positioning of patients
Job Requirements:
5 years of experience required
Professional internal medicine/general medicine experience and/or palliative/hospice experience is preferred
Current PA state license and certification are required (DO or MD)
National certification eligible for licensure by applicable Pennsylvania board
Current CPR certification is required
Act 34 Clearance required
