The Outreach Navigator is a dedicated advocate and support professional in navigating the complexities of healthcare and social services who has experience with HIV Hepatitis C or HIV Pre-exposure Prophylaxis (PrEP) or has been affected by substance use disorder. This role focuses on enhancing the quality of life for patients by providing comprehensive support including healthcare coordination education and linkage to essential services. The Outreach Navigator will assess the needs of at-risk patients to provide testing services linkage and/or referral. The Outreach Navigator works collaboratively with healthcare providers pharmacies insurance social service agencies and community organizations to ensure patients receive timely appropriate holistic care. The Outreach Navigator will support outreach activities through patient recruitment and screening.
KNOWLEDGE SKILLS AND ABILITIES:
- Knowledge of the cultural and social characteristics of the Native American and ethnic minority populations in Maricopa County.
- Knowledge of the economic and health constructs for Native Americans and ethnic/ sexual minority populations in Maricopa County.
- Knowledge and experience working with individuals affected by or at high risk of developing HIV Hepatitis C or other sexually transmitted infections.
- Culturally responsive to ethnic and sexual minorities.
- Comfortable talking about safer sex practices and familiar with harm reduction best practices.
- Experience conducting Rapid HIV testing or its equivalent.
- Experience drawing blood or willingness to become a certified Phlebotomist.
- Must possess excellent Communication skills.
- Ability to multi-task.
- Must be able to work in an outreach setting and support mobile unit activities as needed which includes lifting 20 lbs. above your head.
- Be a detail-oriented professional and effective leader who can work well with other healthcare personnel and interact in a culturally responsive way with patients and their families.
- Knowledge of client confidentiality and client rights according to the Health Insurance Portability and Accountability Act (HIPAA).
- Ability to effectively function as a team member as part of NATIVE HEALTH.
Qualifications :
- Must have a high school diploma or equivalent; an associates degree or higher in social work is preferred.
- Must be either a certified phlebotomist or willing to become certified.
- Must have at least one (1) year of experience navigating the HIV health system in Maricopa County.
- Must have experience navigating and working in a clinical setting.
- Must be familiar with working in a team environment.
- Being bilingual in Spanish is preferred but not required.
- Experience working with Electronic Health Records (EHR) systems and other relevant software programs for documentation scheduling and communication.
- Ability to accurately enter and retrieve data while maintaining confidentiality and HIPPA compliance.
- Flexible work hours may be required during evenings and weekends.
Standard Employment Requirements
- Must possess a valid and unrestricted Arizona drivers license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona drivers license during employment. Must be insurable under NATIVE HEALTHs liability auto policy.
- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an updated immunization (IZ) record at the time of employment.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
- Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.
