As a part of our Foster Care Program you ll join a dedicated team committed to making a lasting impact in the lives of vulnerable youth and the families who care for them.
The Foster Care Case Manager is responsible for providing case management services to children placed in foster care and their foster families. This role involves coordinating services conducting home visits supporting foster parents ensuring compliance with state and agency regulations and advocating for the well-being and permanency of each child.
Manage a caseload of children placed in foster homes.
Conduct regular home visits and document observations and interactions.
Develop monitor and update case plans in collaboration with children biological families and foster families.
Coordinate services such as counseling medical care and educational support.
Facilitate communication between all parties involved in the child s care (birth families foster families schools therapists legal representatives etc.).
Attend and participate in court hearings family team meetings and case reviews.
Provide training guidance and support to foster parents.
Ensure compliance with state licensing requirements and agency standards.
Maintain accurate and timely documentation in case files and electronic systems.
Be available for crisis support and participate in on-call rotation as needed.
Bachelor s degree in Social Work Psychology Human Services or related field (Master s preferred).
Minimum 1 2 years of experience in child welfare foster care or a related field.
Knowledge of child development trauma-informed care and family dynamics.
Strong organizational and time-management skills.
Excellent verbal and written communication skills.
Ability to work independently and collaboratively in a team setting.
Valid drivers license reliable transportation and willingness to travel locally for home visits.
Ability to pass background checks and meet state licensing requirements.
Familiarity with foster care licensing regulations and child welfare policies.
Bilingual (English/Spanish) is a plus.
Proficiency in case management software and Microsoft Office.
