We have an exciting opportunity to join Our SinglePoint of Access Team.
Our SinglePoint of Access service is a newly developed team at the hospice and this is an exciting opportunity to be a part of its ongoing growth and development. As a key member of the team you will play a vital role in ensuring that patients receive the right care at the right time.
This service acts as the central point for external referrals into the hospice assessing urgency and need to support seamless patient care. Working closely with the wider clinical teams we strive to maintain patient flow and deliver a responsive high-quality service. It is a growing team and you will have the opportunity to contribute to ongoing service developments and ideas.
About the Role
Hours: 37.5 per week Monday to Friday 9am until 5pm
You will work alongside a clinical assessor and care co-ordinators to prioritise and manage referrals liaising directly with patients families and referrers to ensure timely and appropriate support. You may respond to advice calls offering professional guidance to patients carers and healthcare professionals while signposting them to relevant services as needed and appropriate.

About You
We are looking for a highly motivated and compassionate Registered Nurse (RN) or Allied Health Professional (AHP) who thrives in a dynamic environment and can confidently manage and reprioritise their workload. You will be proactive empathetic and work collaboratively within a multidisciplinary team to deliver the highest standard of care.
With a keen eye for detail and excellent communication skills you will be able to assess and respond to individual patient needs effectively. Strong organisational skills an approachable nature and the ability to engage well with colleagues and external partners are essential for this role.

About Ashgate Hospice
Ashgate Hospice based in North Derbyshire offers free palliative and end of life care for adults with life-limiting or incurable illnesses including cancer neurological diseases and advanced heart kidney and lung conditions. Last year we cared for over 2400 people. Our aim is to support our patients by keeping them informed managing their symptoms and improving their quality of life from diagnosis onwards.
We also provide vital support to the people who are important to them offering practical assistance and emotional support including bereavement services.
