Overview
SHIFT: 8:00am-4:30pm Mon/Tues/Thurs/Fri
Assists with providing homemaker and/or personal care assistance in accordance with established policies and procedures and in support of related public health programs. Performs the direct service tasks assigned in the plan of care established by the RN and coordinates care with other disciplines.

Responsibilities
Clinical Excellence/Care Delivery
Home Care Aide Services
Provide physical support to assist clients to perform daily living activities such as bathing dressing toileting standing walking or light exercise.
Reminds clients to take medications.
Uses safety measures to prevent incidents and instructs clients in home safety measures.
Notifies RN on same day of changes in clients condition and documents notification.
Demonstrates competency in all skills assigned by RN.
Gives clients information on community resources and notifies supervising RN of action.
Encourages clients to complete tasks as able for themselves.
Homemaker Services
Completes assigned household tasks in courteous efficient manner.
Complies with clients needs in completing tasks according to their instructions.
Observes other tasks that could contribute to clients safety and makes suggestions to RN regarding care plan.
Clerical Support
Provides clerical support for other public health programs by answering telephone calls making appointments entering data into computer systems assisting with equipment requests and maintenance of loan closet.
Qualifications
Education:
High School diploma GED or high school equivalent.
Certified Nurse Assistant (CNA) 75-hour course preferred.
Experience:
Home Care Aide experience preferred.
License(s)/Certification(s):
Valid drivers license when driving any vehicle for work-related reasons.
