Job Description
Blanchard Valley Health System in Northwest Ohio is seeking a compassionate and experienced Nurse Practitioner with a strong commitment to palliative care hospice and geriatrics. Join a community-based nonprofit that truly values its providers and their impact on patient lives.
Job Description:
- 75% Inpatient Palliative Care; 25% Outpatient Palliative Care
- Delivering comprehensive Inpatient and Outpatient Palliative Care services at Lima Memorial Hospital (LMH)
- Conducting patient consultations for symptom management chronic disease management advanced care planning and hospice transition discussions
- Leading delicate conversations with patients and families regarding terminal diagnoses and care goals
- Performing inpatient rounds for palliative care and hospice patients including ICU rounds
- Managing outpatient palliative visits through Oncology department infusion center Comprehensive Health Clinic and Telehealth
- Collaborating with hospital specialists (ICU Oncology Pulmonology Cardiology) to align care plans
- Providing independent care in a fast-paced hospital environment with backup from Bridge team in Findlay as needed
- Maintaining documentation in Meditech (inpatient) and eClinicalWorks (outpatient)
- Serving as a clinical expert in palliative care educating staff and advocating for patient-centered care
- Managing up to 6 patient encounters per day over a 5-day workweek
- Not accepting Locums candidates
- Not accepting new graduate candidates - experience required
- Competitive base salary based on experience and training (to be discussed at interview) with quality bonus potential. Provider may submit salary expectations with presentation.

- Board Certified NP - REQUIRED
- OH license (or willing to obtain) - REQUIRED
- DEA (must be obtained prior to start) - (note in Additional Submission Details field) REQUIRED * no exemptions
- Experience as advanced provider in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) - REQUIRED
- Clean malpractice and license - REQUIRED
- Date of agencys last communication with provider (put answer in Additional Submission Details) - REQUIRED
