About the Position:
This position is responsible of the coordination of services for adults and children with developmental disabilities who live on their own with their families or in a foster care or group home setting. This includes conducting assessments developing and facilitating service plans monitoring of plan-related services and resource referrals.
This position may be eligible for an alternative and/or hybrid work schedule.
