We are seeking a Nurse Practitioner OR Physician Assistant to:
- Conduct comprehensive health history and physical assessment of patients referred with hematologic abnormalities cancers /- family history of the same and/or known or suspected hereditary blood disorder or cancer predisposition syndrome.
- Perform procedures required for evaluation and treatment such as: skin punch biopsy bone marrow biopsies with/without conscious sedation.
- Conduct longitudinal follow-up and early detection and prevention for patients and families with increased risk of bone marrow failure or blood or other cancers.
- Primarily manage and conduct longitudinal follow up for patients with hematologic conditions (ie. anemias hemochromatosis).

Education:
Minimum
If Nurse Practitioner:
Masters or Doctorates degree from accredited Nursing Program.
If Physician Assistant:
Bachelors or Masters degree from accredited PA Program.
Licenses and Certifications:
Minimum -
If Nurse Practitioner: Active RN and APNP license in State of Wisconsin AND active board certification as a NP in Family.
If Physician Assistant: Active PA license in State of Wisconsin AND active PA board certification.
All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire AND CPR certification and advanced life support certifications if relevant to practice area (ACLS PALS NRP).
