Description

JOB TITLE: Individual & Family Services (IFS) Outreach Coordinator


I. Minimum Education/Experience:

  1. Bachelors degree in Social Work Psychology or related field with at least one year of experience providing case management to adults children and families.

II. Responsibilities:

  1. Administer pre- and post-tests for data collection at partner agencies following grant requirements.
  2. Administer Developmental Assessments to assigned children for partner agencies and the First Steps Assessment Team.
  3. Conduct First Steps Intake and scheduling of assessments as needed.
  4. Participate in weekly one-on-one meetings with your supervisor and monthly team meetings as needed.
  5. Maintain an outreach list of clients from partner agencies and conduct monthly check-ins for a total of 6 months after they leave the partner programs.
  6. Maintain a working knowledge of current Wake County agencies and programs to facilitate transitions and appropriate referrals as needed for clients.
  7. Connect families to community resources through case management (i.e. Social Determinants of Health Housing Security Food Security Economic Security Education Healthcare Transportation Interpersonal Violence and Toxic Stress).
  8. Perform other related duties as needed.

III. Recruitment Standards:

  1. Ability to relate well with and understand the needs of all members of the families enrolled. This involves skill in transmitting information to parents building a rapport with many different families.
  2. General understanding of child development socioeconomic needs and family dynamics.
  3. Ability to establish and maintain accurate records and to work with the Lead Therapist for quality control.
  4. Ability to lift to 40 lbs.
  5. Ability to drive to all areas of Wake County as needed.

IV. Hours/Days of Work:

Full-Time; hourly 40 hours per week (non-exempt); Weekly schedule must be flexible to meet the needs of families served including some evening and/or weekend meetings.

Psychologist / Therapy Services Manager

$21.00 hr.
