Your Opportunity
As a Youth Case Manager you will deliver person-centred strengths-based support to young people and their families to help them make positive sustainable changes. Working collaboratively you will guide clients through referrals and individualised planning to access housing employment education and wellbeing services.
This is an on-site position based in Darwin where you will provide holistic culturally responsive case management aimed at breaking the cycle of homelessness and disadvantage and supporting young people to thrive in their communities.
Your Key Responsibilities
- Respond to internal and external referrals including those from FDV and AOD services and assess suitability for support.
- Conduct comprehensive face-to-face assessments using approved tools and complete associated documentation.
- Develop holistic culturally responsive case plans in partnership with clients to support wellbeing housing and independence goals.
- Maintain clear professional boundaries while supporting clients to build life skills and capacity.
- Provide ongoing formal and informal case management and regularly review individual progress.
- Connect clients with appropriate supports including health education AOD FDV social and community services.
- Complete case management duties including referrals support letters appointment coordination and advocacy across service systems.
Requirementsfor Success
- Degree in Community Services (or related field) with at least one years experience or Associate Diploma with relevant experience or equivalent practical experience.
- Understanding of issues impacting homelessness including AOD FDV mental health and unemployment.
- Demonstrated experience in case management including assessments planning advocacy and accurate record-keeping.
- Knowledge of and ability to build networks with housing accommodation community services and government agencies.
- Experience working with diverse client groups particularly youth and families affected by complex issues.
- Strong verbal and written communication skills with digital literacy across Microsoft Office and case management systems.
- Current First Aid and CPR Certificate.
- Full driver licence with no restrictions
Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
