Sentara Health Plansis hiring a Temporary Care Coordinator for the Northern Virginia area!
Status: Temporary (Minimum of 3 months with possibility of extending)
Hours: Full Time (40 hrs/wk)
Shift: Day (8am-5pm)
Pay: $35.84/hr
This position requires travel for in person face-to-face assessments on average 3-4 days per week. Days without assessments scheduled employee can work remotely.
Applicants must be located in Northern Virginia or Surrounding area
Area includes: Fredericksburg Arlington Woodbridge Culpeper Spotsylvania Louisa Rapphannock Prince Edward Farmville Manassas
The Care Coordinator performs a variety of casework duties and provides case management services to patients families and designated caregivers. Must develop participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses
Bachelors Degree (Required)
Bachelors Degree must be in Health & Human Services or one of the following related Fields:
-Art Therapy
-Behavioral Sciences
-Child Development
-Cognitive Sciences
-Community Mental Health Counseling (MH Vocational Pastoral etc.)
-Counselor Education
-Early Childhood Development
-Educational Psychology
-Gerontology
-Healthcare Administration
-Human Development
-Human Services
-Marriage and Family Therapy
-Music Therapy
-Nursing
-Pharmacy
-Psychiatric Rehabilitation
-Psychology
-Rehabilitation Counseling
-Social Work

1 year of Long Term Care and Health Plans experience (Required)
Nursing Home Experience (Preferred)
Case Management/Care Coordination experience (Preferred)
Experience with Community Health Resources (Preferred)
Experience initiating care plans and both subjectively and objectively conducting assessments (Preferred)
