Job Scope: This position provides direct-service to individuals and families looking to access intensive mental health services and support to youth with a severe emotional disturbance. The service is available to children and young adults under the age of 26. The role has a primary focus on helping families gain access and authorization for the Family Support Program as well as maintaining the ongoing availability of those services. The secondary part of this role includes providing support education skills training and case management to the parents of community based MCR and C&A outpatient clients. Participates as a member of the CCSO team. Program Responsibilities: - To identify family needs that pose barriers to a clients clinical goals and to connect the family with resources to meet those needs.
- To educate families in an individual or group format regarding mental illness and the systems that treat mental illnesses.
- Build skills in the parents to effectively parent children with mental illness while managing a household effectively in individual or group settings.
- To collaborate with other members of the treatment team.
- To assist potential FSP clients/families with completion and submission of the FSP application.
- To provide intensive care coordination case management assessment and treatment planning to FSP families including residentially placed clients.
- Serve as the SFSP coordinator for 90 days of crisis stabilization community mental health and assessment services to youth at risk of custody relinquishment.
Clinical Responsibilities: - Provide support to families through identification of pre-existing resources and connection to community based resources to meet unmet needs and reduce the risk factors impacting the client/family
- Provide case management to FSP eligible youth and their families. The CCSO must make direct contact (telephonic or in-person) on a weekly basis with the FSP eligible youth the youths legal guardian and the youths residential treatment provider when applicable throughout the youths enrollment in FSP.
- While the youth is receiving residential treatment the FSP Coordinator must continue to provide case management and support to the youth and family including the following:
- Actively participate in all residential staffing and discharge planning meetings;
- Maintain weekly contact with the youth family and the RTF;
- Coordinate the provision of mental health and other supportive services to the family in preparation for supporting the youth following discharge;
- Maintaining the IMCANS in coordination and with input from the RTF to ensure continuity of services for both the youth and family
- Support the youths transition out of the RTF.
- Develop and regularly review and update a Crisis Prevention Safety Plan for all FSP eligible youth. The initial CPSP must be established or reviewed and updated as appropriate within ten (10) days of the youths FSP enrollment. The CPSP must be reviewed and updated following an MCR event or minimally once every 180 days.
- Provide comprehensive initial Integrated Assessment and Treatment Plan review and updates (IMCANS) to community based clients.
- Collaborate with clients and their families in the development of treatment plan goals objectives and interventions reflecting clients preference for care and demonstrating medical necessity
- Provide more intensive linkage and support when approved by the supervisor.
- Educate families about SFSP identify and link eligible youth and families to the services and supports needed to stabilize the family in crisis and make recommendations to the state regarding on going services needed to support the youth.
- For SFSP enrolled youth develop and support a plan for safe transition of youth back to home or community setting by collaborating with the discharging hospital treatment team parent/guardian and youth.
- The SFSP Coordinator is responsible for contacting the youth the parent/guardian and the staff from any out-of-home treatment setting as applicable no less than once per week.
- Develop SFSP Assessment Report and submit to the Interagency Clinical Team within 75 days of youths SFSP referral.
- Participate in supervision as directed with the Manger of Pathways
- Represent agency at state required meetings for FSP
- Provide comprehensive mental health assessment that identify service needs
- Participate in consultation as needed with other agency professionals or with paid agency consultants.
- Participate in team meetings as directed.
- Gather organize and become familiar with and utilize outside resources to provide comprehensive services to clients.
- Work cooperatively with other staff to maintain smooth continuity of care when cases are shared across teams.
- Participate with the Supervisor in regular performance evaluations
Education and Experience: - Bachelors Degree from an accredited College or University in Social Work Psychology or closely related discipline
- Minimum of 4 years experience working with children and adolescents
- Valid drivers license proof of insurance and reliable transportation required
- Computer skills required
- (IMCANS) preferred
Work Environment - Roughly one half of the employees time will be spent in the community driving to and from family homes and schools. The other half will be in office time.
Schedule: - ExemptFull-Time 37.5 hour work week B. Must work a flexible schedule in response to the needs of the target population served. This schedule may include evening hours.
