Your Role
The Integrated Health Manager will lead care coordination efforts across collaboration with organizational leaders they will develop the vision for care coordination in the organization. This position will identify trends and monitor related performance indicators with the goal of improving health outcomes for CMHC consumers. They will coordinate and facilitate psychiatry services for consumers through CMHC. They will serve as Sertomas lead clinical liaison to managed care organizations (MCOs) Integrated Health Homes (IHH) and The Illinois Health Practice Alliance (IHPA). This position will provide supervision for team members and will be responsible for the day-to-day operations of care coordination services implemented throughout the organization.

Responsibilities
  • Carry out supervisory responsibilities in accordance with the organizations policies and applicable laws. Responsibilities include but are not limited to recruiting onboarding and training employees; performance management; and conflict resolution.
  • Organize and direct care coordination team efforts across departments and programs to ensure standards are met. Communicate with various partners and stakeholders across the organization.
  • Oversee the Integrated Health Team for all Colbert Class Members who are assigned to Sertoma Centres Prime network.
  • Oversee the Health Educator (LPN) to ensure health services are being provided per the Northwestern Medicine Community Benefit Grant.
  • Identify member needs relative to medical and behavioral health through consultation and supervision with Integrated Health Team and develop practices to identify and fill gaps in member service.
  • Direct responsibility for management of Sertoma Centres IHPA Program. Ensure interventions assigned through HealthEC and Bamboo Health databases are completed; lead participant in all IHPA conferences calls and meetings; ensure that IHPA services are properly billed pursuant to Medicaid. Develop standards of practice relative to the delivery of IHPA related services.
  • Enhance tracking systems to find those disconnected from treatment through review of claims and initiate contact via Integrated Health Team.
  • Ensure class members transitioning out of nursing facilities receive necessary care coordination services prior to their move.
  • Evaluate programs effectiveness in meeting deliverables. Recommend modifications in service structure or operations that affect the program. Develop plans to improve program performance.
  • Coordinate and manage psychiatry services across CMHC including scheduling appointments managing the waitlist (if applicable) communicating with CMHC staff and consumers about availability of appointments and working directly with the assigned psychiatry provider and liaison through Array Behavioral Care.
  • Oversee the completion and submission of data tracking and reporting by the Health Educator (LPN) as it pertains to the specified outcomes for the Northwestern Medicine Community Benefit Grant.
Qualifications
  • Bachelors degree in human services or related field; Masters degree preferred.
  • Knowledge and/or experience in mental health services and integrated health.
  • Minimum of one year supervisory/administrative experience.
  • Minimum of one years experience working with individuals with psychiatric disorders and working knowledge of the recovery model.
  • Three years of case management care coordination linkage outreach and/or community support experience.
  • Ability to work in a variety of environments and willingness to provide services in location most
    convenient to the individual served.
  • Effective communication skills both oral and written.
  • Valid drivers license and documentation of current auto insurance with an acceptable motor vehicle report.
  • Proficient in the use of computers software applications and working knowledge of Microsoft Office Suite programs.
