Position Overview:

Residential Transitions Inc. (RTI) is seeking a dedicated Waiver Case Manager for Dakota & Ramsey Counties. We work with people integrated into the community living in their own homes and in our residential settings. Our mission is to help people improve their quality of life by empowering them with knowledge resources and skills and treating them with dignity and respect. Waiver case manager provides person-centered case management services to individuals diagnosed with serious and persistent mental illness substance use disorders and co-occurring medical conditions who are eligible to receive services through the Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waiver Case Management services through Hennepin County. This position requires in-person training at our main office for the first 90 days.

Role Responsibilities:
  • Manage and maintain a caseload of individuals who are diagnosed with serious and persistent mental illness substance use disorders and co-occurring medical conditions throughout Dakota & Ramsey Counties
  • Visit clients at least once a month and twice annually in accordance with waiver-specific rules.
  • Coordinate client intake and discharges within set parameters and timelines.
  • Collaborate with other professionals such as physicians mental health providers and other teams as needed to develop and update plans for each client.
  • Ensure financial authorizations and provider contracts are in place
  • Ensure services are documented in individual records so information is readily available results of services can be determined and record-keeping standards are met.
  • Access available community and housing resources so that necessary and relevant resources match an individuals needs and preferences.
  • Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community.
Required Qualification and Experience:
  • The Waiver Case Manager shall be qualified under Minnesota law as follows:
    • Holds a bachelors degree in one of the behavioral sciences or related fields from an accredited college or university and has 2000 hours (1 year) of experience in the delivery of service to persons with mental illness Or masters degree in behavioral science or related field.
  • Willingness to travel within the Metro Area for applicable mileage reimbursements with a current drivers license clean record and personal vehicle with insurance; and able to transport clients in that vehicle.
  • A valid Drivers License reliable vehicle satisfactory driving record and auto insurance are required
  • Must be able to lift and carry items up to or exceed 40 pounds.
  • Have a working knowledge of person-centered planning harm reduction and the needs of individuals with SPMI/CD.
  • Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.
Preferred Qualification and Experience:
  • Masters degree in behavioral science or related field
  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings
