Health Access Program at CHN: The Health Access team at CHN contracts with the Colorado Department of Public Health & Environment (CDPHE)s office of STI/HIV/VH to carry out certain aspects of the State Drug Assistance Program (SDAP). This program prioritizes improving access to health care for individuals living with HIV who qualify for SDAP program benefits. The statewide Health Access team works directly with clients to facilitate medical insurance enrollments navigate challenges in the US Health Care System and provide financial assistance for qualifying medical expenses.
Position Summary: The Health Access Aging Services Coordinator plays a key role in supporting older adults living with HIV by facilitating health insurance access and related healthcare services. This position maintains an active caseload of 100 clients over the age of 50 and provides one-on-one assistance with Medicare Medicaid private insurance enrollment and Social Security Disability Insurance (SSDI) navigation. Due to the nature of health insurance navigation this position has notable shifts in demand based on season with General Open Enrollment Periods at the end of each year requiring the highest volume of work. The Coordinator works closely with CHN departments community partners and state agencies to ensure smooth client transitions and warm handoffs.
This role is ideal for a professional with a strong understanding of aging-related issues HIV/AIDS Medicare and public health systems. The successful candidate will be organized compassionate and adept at helping clients overcome complex barriers to care.
Performance in this position is measured through formal supervision electronic data analysis and routine quality assurance auditing.

Qualification:
Education:
Bachelors Degree in Social Work Human Services Public Health Gerontology and/or a related field or equivalent experience required; candidates with lived experience can substitute educational background.
Software Utilized:
Experience working with basic office and database computer programs including familiarity with Microsoft Outlook Word Excel Access SharePoint and social media outlets required.
Material and Equipment Directly Used:
Works with standard office equipment including computer workstation printers copiers scanners and fax machine. Works with word processing software spreadsheet applications and presentation software. Communicates with clients and vendors using the telephone e-mail the Internet and fax.
Physical Demands and Work Conditions:
Works in an office environment. Will travel to offsite locations. Regular weekly and monthly reimbursable travel required. Requires ability to lift up to 50 pounds of materials. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires fingering agility for keyboarding and using a telephone and other office equipment.
