Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Providing skilled nursing care to patients with advancing age frailty and serious illness.
- Performing patient assessments and developing treatment plans.
- Administering medications and treatments.
- Collaborating with physicians other nurses and other members of the healthcare team.
- Case management and hospice care.
- Maintaining accurate patient records.
- Educating patients and families on health management.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Two years of minimum experience in hospital home health or hospice care.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- The role may involve working in a fast-paced environment and requires significant patient interaction. Field work requiring use of a personal vehicle is necessary. The nature of hospice and home health care may require irregular hours or on-call responsibilities.
