Position Summary
The Youth and Young Adult (YYA) Case Manager supports individuals ages 1824 in the agencys Youth and Young Adult Transition in Place (YYA TIP) program. As a member of the Housing & Shelter team the Case Manager provides comprehensive case management servicesincluding housing navigation income development and connections to behavioral health and medical careto young adults transitioning out of homelessness. This role is dedicated to promoting housing stability and self-sufficiency with the ultimate goal of ensuring that homelessness is brief rare and nonrecurring.

Qualifications
Our ideal candidate has a High School Diploma or GED; Bachelors Degree preferred. Some experience working with people experiencing homelessness veterans people with substance use disorders people with re-entry backgrounds and/or other relevant social service populations preferred. A Valid Ohio Drivers license and proof of insurance required
