Blended Case Management Supervisor
Position Description:
We are looking for a Blended Case Management Supervisor to join our Team!
Nulton Diagnostic & Treatment Center is accepting applications for a Blended Case Management Supervisor for our Cambria County teams. The successful candidate will provide Blended Case Management Services and Supervision in accordance with Federal State and Agency requirements.
Education Experience Skills Training:
The following minimum requirements shall be met by supervisors of blended case management services:
A. A masters degree in social work psychology rehabilitation activity therapies counseling or education and 3 years mental health direct care experience; or
B. A bachelors degree in sociology social work psychology gerontology anthropology history criminal justice theology counseling education or be a registered nurse and 5 years mental health direct care experience 2 of which shall include supervisory experience; or
C. A bachelors degree in nursing and 3 years mental health direct care experience.
Annual Bonus of up to $10000.
The successful applicant must have a valid Drivers License and be able to obtain Act 33 Act 34 and FBI clearances.
This is a full-time salaried position which includes training competitive salary use of company vehicle and medical dental and vision benefits as well as paid vacation and paid holidays. This position also offers Incentive-Based Bonuses.
