ESSENTIAL DUTIES FUNCTIONS AND RESPONSIBILITIES
- Coordinates with the team on assignments projects problem areas to be solved actionable events milestones program issues under review deadlines and time frames or completion.
- Distributes and balances the workload among employees in accordance with established workflow. Assures timely accomplishment of the assigned workload.
- Provides on the job training to team members in methods and techniques of team building and working in teams to accomplish tasks or projects and provide or arrange for specific administrative or technical training necessary for accomplishment of individual and team tasks.
- Instructs employees in specific tasks and job techniques and ensures all competencies are completed and documented.
- Reports on expected time of completion of work and maintains records of work accomplishments and time expended and prepares reports as requested.
- Represents the team while working with the Chief of Pharmacy for the purpose of obtaining resources and securing needed information or decisions on major work problems and issues that arise.
- Collaborates with the Chief of Pharmacy in the performance evaluation of team members including work accomplishments problems progress in mastering tasks and work processes and individual and team training needs.
- Researches learns and applies a wide range of qualitative and/or quantitative methods to identify assess analyze and improve team effectiveness efficiency and work products.
- Approves leave as needed when Chief of Pharmacy is not available.
- Resolves simple informal complaints of employees and refers others to the Chief of Pharmacy. Keeps Chief of Pharmacy informed of informal complaints resolved.
- Keeps employees informed of procedures policies and directives.
- Informs the Chief of Pharmacy of performance management issues/problems and to recommend/request related actions.
- Assists with scheduling of outpatient pharmacy staff. Delegation of projects and responsibilities to staff.
- Manages daytoday outpatient pharmacy operations with supervision of pharmacists pharmacy technicians and pharmacy support staff.
- Reviews inpatient pharmacy-related policy annually and is responsible for policy maintenance and/or development as related to pharmaceutical services facility wide.
- Participation in pharmacy and hospital administrative meetings as assigned.
- Ensures annual evaluations of outpatient pharmacyrun clinics and/or services are completed.
- Evaluates the impact of pharmacy services through a planned program of quality assurance that meets the standards of the Center for Medicaid and Medicare Services.
- Evaluates and interprets culture and sensitivity reports recommends appropriate antibiotics and participates in antimicrobial stewardship.
- Serves as therapeutic consultant and source of drug information for medical staff and nursing.
- Performs outpatient pharmacy functions for the full range of pharmaceuticals provided in the medical facility that require special handling.
- Reviews medication records history and physicals outpatient progress notes labs and additional information pertinent to patient care for accurateness efficacy potential for drug interactions potential for adverse reactions appropriateness and timeliness of treatment for patients. Reviews the use of nonformulary and/or restricted items for appropriateness and/or approval by the Pharmacy and Therapeutics committee.
- Compounds or oversees compounding of medications as prescribed either individually or in bulk. Determines validity of formulations dosage forms and quantities of dosage forms to be used to prepare desired concentration and quantity and will assign appropriate expiration dating. In addition will research at the request of providers compounding recipes for special compound orders.
- Inspects processed medications for integrity of dosage form; correct labeling handling and storage instructions.
- Conducts necessary patient education concerning the medication its administration signs of reaction and additional relevant materials per IHS standards concerning patient counseling (i.e. ShowandTell method).
- Counsel patients on course of treatment and indication for treatment as necessary. When prompted by patients will provide disease state education to further educate patients on management and treatment options.
- Performs monthly inspections of assigned drug storage areas.
- Ensures proper equipment treatment and maintenance.
- Ensures equipment is properly setup and operated.
- Provides drug information to providers nursing family and other healthcare providers utilizing hardcopy online and other drug reference materials as necessary and appropriate.
- Reports and documents medication errors and/or interventions per FDIHB Pharmacy policy.
- Participates in pharmacy learning activities as presented to staff by residents students and other pharmacy staff.
- Oversees all outpatient pharmacy technician activity as listed in the pharmacy technician position description.
- Assists with the training of new staff. Acts as preceptor when assigned a student.
- Represents the pharmacy department on various committees and at meetings as assigned by pharmacy administrative staff.
- Supports clinical investigations as required.
- Acts as technical support to the best of their knowledge for providers pharmacists nurses and additional hospital staff for CPOE and/or EHR.
- Works with anticoagulation lead Pharmacist to ensure that all appropriate guidelines are followed.
- Performs other duties as assigned
MINIMUM MANDATORY QUALIFICATIONS
Experience:
- Five (5) years of outpatient pharmacy experience and one (1) year supervisory experience in an
outpatient pharmacy.
Education:
- Completion of a Pharm. D. Program from an accredited college or university.
- Must maintain an active unrestricted Pharmacy License in any U.S. State or Territory by
completing necessary Board of Pharmacy requirements for the respective state of licensing.
