Were currently seeking a passionate and collaborative team player to join our Connecting Home service supporting individuals who are experiencing or at risk of homelessnessspecifically focusing on mature-aged participantsacross Lismore and Grafton areas.
You will play a key role in helping participants access safe sustainable housing solutions and connect with the supports they need. No two days will be the samewhether youre offering phone-based support conducting outreach or working alongside other community-based services to deliver strength based person-centred care.
