As a growing organisation we are seeking a dedicated and organised Care Coordinator (Full-Time) to join our team. This role is ideal for someone who thrives in a fast-paced environment enjoys problem-solving and is passionate about improving the lives of people with disabilities.
As a Care Coordinator you will be responsible for overseeing the day-to-day coordination of individual support services. You will ensure that client needs are being met through the effective scheduling of support staff monitoring of care delivery and communication with clients families and stakeholders.
