Job Description

Job Title: Community Violence Intervention Coordinator
Reports to: Chief Program Officer Violence Prevention
FLSA Code: Full time Exempt
Hours: 40-hour workweek; occasional weekends and evening hours as needed
Location: Mens Center 402 N. St. Louis Ave Chicago IL 60624


POSITION SUMMARY
SC2 aims to scale a CVI (Community Violence Intervention) approach that encompasses a holistic suite of core services for individuals at highest risk of engaging in gun violence. The approach is community-led customized data-driven and will rely on active collaboration among neighborhood stakeholders. The SC2 Coordinator supports the development and
effective implementation of the Garfield Park Scaling Plan. The Coordinator in partnership with Breakthrough staff will ensure effective communication administration and support of partners and Breakthrough to accomplish the goals established by the collaborative. Community partner engagement and collaboration are key aspects of this position. The Coordinator is responsible for activities including but not limited to data processing and analysis records management the development of reports and project will manage partnerships with service providers who carry out the program work develop best practices and peer sharing among partners and oversee partnerships subcontracting agreements with an eye toward system performance measures to effectively implement the Greater Garfield Plan.

ESSENTIAL RESPONSIBILITIES/FUNCTIONS
  • Organizes plans coordinates program operations stakeholder communication community engagement and community outreach ensuring collaboration across SC2s core service areas.
  • Serve as Breakthroughs SC2 liaison for SC2 Community Partners implementation partners and Vendors for communication reporting and planning.
  • Collects documentation and data related to financial vouchers and other funder requests from SC2 community partners.
  • Manages administrative tasks associated with SC2 including co-creation of committee agendas note taking and follow up.
  • Processes vouchers and invoices
  • Participates in community engagement and event planning around CVI.
  • Acts as a liaison to the contracted consultants and implementation partners.
  • Collaborates on the grant writing and fundraising components of SC2.
  • Serves as the referral administrator for Outreach organizations to support service providers.
  • Partners with Breakthroughs Data Specialist and other community partners for data collection.
  • Monitors workflow to identify areas for improvement; support internal and external workflow development and maintenance of workflow and reporting process documentation.
  • Participates in program compliance reviews of community partner agencies.
  • Monitoring and evaluation:
  • Maintains all relevant data for use in tracking analyzing and reporting
  • Performs site-visits to evaluate and support partner performance
  • Responsible for contributing accurate data for funders reports
  • Community Partner Agencies Support
  • Coordinates and leads collaborative community partner agency meetings
  • Gathers and distributes resources for and from community partner agencies
  • Relationship Management
  • Builds and sustains relationships with colleague community leaders and relevant organizations
  • May be called to represent Breakthrough at public and private events
  • Supports initiatives as assigned by the Executive Director.
  • Position responsibilities and work volume will be established and evaluated through ongoing supervision annual reviews and the development of an annual professional development plan.

QUALIFICATIONS
  • Bachelors degree in social services or a related field OR minimum of 1-3 years of experience in human services or a related field OR a combination of related college coursework and related work experience.
  • Knowledge of the westside is required
  • Proven ability to work independently take initiative and manage multiple tasks effectively.
  • Excellent interpersonal public speaking and organizational skills
  • Strong organizational skills and proven attention to detail
  • Ability to work well independently as well as a member of a team
  • Ability to work in a fast-paced self-directed environment

  • Crisis intervention training or knowledge

  • Ability to understand and follow safety/security practices and work in high pressure situations
  • Project a professional image to all internal and external customers ensuring correct grooming
  • Ability to demonstrate initiative and follow through and be comfortable working in a changing environment
  • Demonstrates willingness to consistently improve work performance through their relationship and guidance.
  • Must possess an accurate and informed understanding of cultural and urban realities especially as related to youth families poverty and community development.
  • Ability to develop strong relationships with diverse groups of citizens who may have conflicting interests and opinions.

  • Direction received from the Chief Program Officer Violence Prevention
  • Does not supervise employees

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
Work Environment
While performing the duties of this job the employee will work in a multi-purpose facility and/or outdoors and thus will be exposed to weather conditions prevalent at the time. The noise level in the work environment is usually moderate but can be high depending on the activities in the facility.

Physical Demands
While performing the duties of this job the employee is frequently required to walk and/or sit; use hands and fingers to feel objects tools or controls reach with hands and arms; balance; stoop; regularly talk or hear. The employee must be able to bend stoop pull lift as well as have overhead extension of arms. Must be able to stand for long periods of time; be able to lift carry or move at least 50 pounds. Must adhere to proper handling and lifting techniques. Specific vision abilities required by the job include close vision distance vision peripheral vision depth perception and the ability to adjust focus.

Equipment Used
The employee will generally use general office equipment including but not limited to telephone computer calculator copier paper shredder paper cutter and filing systems.

Safety Requirements
Prior to the first day of work must complete a criminal background and sex offender clearance.

COMPENSATION
$55000-$60000/annually
