Overview

CHI Health Schuyler opened its doors on October 11 1953 serving the communities of Schuyler Clarkson Howells Leigh and the residents of rural Colfax County in Nebraska. In addition to its 25 critical access beds CHI Health Schuyler offers a wide variety of services to residentschildren teens adults senior citizens friends and neighbors. JOB SUMMARY / PURPOSE

Under the supervision of an RN or Physician the LPN-Clinic functions as a vital member of the healthcare team who contributes to the success the practice by demonstrating customer/patient focus effective communication professionalism and teamwork. Utilizes the nursing process to assess plan implement and evaluate comprehensive care provided to selected patient populations and their families.

ESSENTIAL KEY JOB RESPONSIBILITIES

  1. Performs general nursing care to patients by following established clinic workflows.
  2. May perform specific nursing care as it relates to specialty of the practice.
  3. Obtain patient consent for care and ensure patient confidentiality.
  4. Obtain and document health history information on chief complaint vital signs and health risk factors and assesses disease condition.
  5. Provide patient care based on practice guidelines standards of care and federal/state laws and regulations. Execute prescribed treatments and medical interventions administer prescribed medications and monitors and documents treatment progress and patient response. This includes but is not limited to preparing and administering injections performing routine tests treating wounds and changing bandages. Refer to/consult with physicians other health providers and community resources to prevent/resolve problems or concerns.
  6. Educate patients and families about health status health maintenance and management of acute and chronic conditions.
  7. Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
  8. Participate in multidisciplinary teams to improve patient care processes and outcomes.
  9. Track quality assurance data and monitor for acute and chronic care management.
  10. Performs other duties within scope of role as assigned.

Qualifications

Required Education for Staff Job Levels

  • Graduate of accredited school of practical nursing required.

Required Licensure and Certifications

  • Current/valid Licensed Practical Nurse License from the state licensing board required.
  • Basic Life Support (BLS) or Advanced Care Life Support (ACLS) required.
