Join our clients extraordinary care team as a Hospice LPN in Shiloh/Fairview Heights IL. Key Responsibilities
- Provide management and delivery of patient care plans
- Collaborate with the care team
- Support patients in Stroudsburg PA and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule
- Utilize tablets mobile phones and EMR software
Schedule and Shift Details
- Full Time
- Occasional local travel
Qualifications
- Associate degree or higher
- Hospice (2 years)
- 1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
- Valid Licensed Practical Nurse (LPN) in Illinois (IL)
- Do you have a valid Drivers License Reliable Mode of Transportation and Auto Insurance (all 3)
- Are you okay with the required travel for this position
