Job Overview

Location/Schedule: This position is based out of Lloyd Cooperate Plaza located in NE Portland OR but position will travel to multiple health center locations. The schedule for this position is typically Monday through Friday 8:30 a.m. to 5:00 p.m.

Position Description: The Integrated Team-Based Care (ITBC) Manager is responsible for providing operational and clinical oversight for three ITBC teams and for ensuring the effective functioning of said teams. This position is part of a matrixed leadership structure and is responsible for collaborating with Discipline Supervisor leadership structure to create a robust system of support and accountability for all ITBC staff. The role is the supervisor of record for an administrative specialist and a care coordinator and is responsible for ensuring compliance with regulatory and organizational standards. The TBC Manager works closely with the TBC Director and QM/Population Health to improve outcomes for TBC panel members.

Essential Responsibilities:

This position description is not intended to be an all-inclusive list of responsibilities skills or working conditions associated with the position. Management reserves the right to modify add or remove duties as necessary.

General:

  • Participate in all scheduled staff meetings supervision sessions and other departmental and agency meetings.
  • Oversee the treatment and clinical needs of clients and provide input into treatment process when team members need additional input. Oversees panel goals in collaboration with TBC leadership and assigned TBC team.
  • Assist staff members by helping to resolve difficult client and program situations thereby managing client risk and care.
  • Serve as Leader of the Day (LOD) as scheduled handling client calls and crises.
  • Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.
  • Assist in crisis situations as appropriate following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma-informed environment for clients and staff.
  • Organize daily activities and ensure team follows approved clinical guidelines and operational workflows.
  • Demonstrate responsibility for safety of consumers staff and property; is familiar with Health Center safety policies and procedures.
  • Attends and completes required employee training as assigned.
  • Perform other duties as assigned.

Compliance:

  • Maintain knowledge of review and ensure compliance with all applicable federal state county regulations and internal policies and procedures.
  • Assure that staff members document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance. Monitor daily reports to ensure that operational and clinical documentation is timely.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Maintain supervision notes and various compliance reports.
  • Collaborate with Quality Management/Population Health to ensure compliance.
  • Monitor key performance indicators (KPIs) and metrics as decided by the TBC Leadership Population Health and Executive Leadership.

Supervisory Oversight:

  • Provide supervision and manage employee performance including support plans and disciplinary actions for the Administrative Specialist and Care Coordinator as indicated.
  • Conduct and document individual supervisory session and group supervisory sessions as directed by internal policies and guidelines.
  • Make hiring decisions for administrative staff and care coordinators in collaboration with P&C.
  • Assign daily tasks to team members based on client needs staff availability and best use of each ITBC role assuring that walk-in scheduled and community-based services are available as indicated.
  • Participate in on-call rotation providing after-hours support when necessary.
  • Support the Care Coordinator in monitoring the panel dashboard identifying needed changes in treatment and ensuring the team addresses needs in a timely manner.
  • Collaborate with each team members Discipline Leader to support professional development and accountability for the team.

Leadership:

  • Work collaboratively with The ITBC Director other TBC Managers Discipline Leaders and other clinical leads to develop monitor and evaluate best practices for team functioning including the implementation of Feedback Informed Treatment (FIT).
  • Ensure TBC team meetings and huddles are effective and efficient.
  • Train and coach current and new team members on TBC workflows and standards.
  • Identify and nurture Emerging Leaders providing leadership in developing initiatives and Strength-Based Leadership learning.
  • Using recovery and ITBC approaches to treatment and a whole healthcare lens provide clinical leadership and training/coaching to the assigned TBC Teams.
  • Prepare various reports for the TBC Director or other stakeholders as required/requested.
  • Manage budgets for each team.

Qualifications

Education:

  • Required: Masters Degree in a Behavioral Science field from an accredited college or university.
  • Demonstrated competency to become credentialed as a Qualified Mental Health Professional (QMHP) as defined by OAR. The incumbent is expected to keep certification current and in compliance with the OARs. *or have the ability to obtain within 30 days of hire

Experience:

  • Required: A minimum of 3 years of supervisory experience AND 2 years of program management experience.

Specialized Knowledge Skills & Abilities:

  • Understanding of recovery model motivational interviewing illness management and recovery evidence-based practices and strength-based case management philosophies and strategies.
  • Understand and align with harm reduction philosophies cultural responsiveness and trauma informed care.
  • Knowledge of best practices.
  • Operate computer hardware and software at a level needed to effectively perform job functions.

Other:

  • Ability to commute to other locations.
  • Ability to perform BPR First Aid and effectively use AED equipment. BPR/ First Aid certifications are required and must remain current.

Working Conditions

Environmental:Work is performed in one or more of the following: clinical office acute care settings and community setting including client residences correctional facilities shelters hospitals public places and other social service sites. Meal periods and breaks are subject to interruption. Much of the work is performed during regular business hours with the potential of extending into afterhours only for crisis response. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior exposure to disease biohazards noise and contaminants.

Mental: The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach. In addition this position requires exercising sound judgment. Decision-making may occur in a moderate to high stress situation where a client may be experiencing a crisis.
