Cornwall Councils Children and Family Services are committed to ensuring every child and young person receives the support they need to thrive. The Statutory SEND Service plays a vital role in delivering high-quality person-centred support for children and young people with EHC plans.
The Role:
The role holder will work with other SEN professionals in the Inclusion and SEND service to ensure effective proactive support of families of children with SEND and young people with SEND. The role holder will be on the front line of communication arranging face to face meetings with families taking calls on Teams or the phone from parents/ carers and young people and being the main point of contact for that family until the enquiry or required action is resolved. This includes maintaining regular communication with families to provide progress checks. Also undertaking a check in annually to ensure any details held are still relevant accurate and up to date and to gather feedback on the service user experience. In liaison with colleagues across SEND services this role holder will ensure the synergy database and SEN files are accurate and kept up to date and will chase update and distribute any documents as required. This will also include communicating with families and schools with regard to deadlines triaging and actioning calls and email enquires as the first point of contact and any other appropriate duties as requested by your line manager. Through delivery of SEND support duties the post holder secures the best outcomes for children and young people with SEN. This role requires the post holder to manage and prioritise their own work whilst also being collaborative informative and person centred in all aspects of work
