Job Description:
Ensuring seamless patient care and efficient administrative operations. To support case managers and healthcare professionals by performing a variety of tasks that range from administrative duties to patient interactions. To facilitate and coordinate a safe and appropriate discharge plan while meeting the patients individual needs as well as state and federal regulations. To work collaboratively with multiple other disciplines throughout the organization and community. To manage patient records and coordinate follow1up care. To enhance the patient experience and optimizing the overall healthcare delivery process.
RNCC (RN-CM) SW-CM and SWs in assessing coordinating care and developing and
implementing discharge plans based on patient needs.
2. Documents planning and interventions appropriately and timely.
3. Communicates appropriate information between physicians nursing units administration and other
disciplines to facilitate care transitions to ensure proper patient flow through the hospital system.
4. Identifies any barriers and provides clarity to determine realistic goals for the discharge plan.
5. Coordinate with patients and providers to arrange or facilitate follow up appointments (service1based/specialty services) such as PCP for DME needs and other post-discharge care needs.
6. Maintain accurate and updated patient notes and referrals/packets in Careport or to contacts
outside of Careport.
7. Provide administrative support such as data entry filing document preparation and patient
signatures.
8. Monitor patient progress and report updates to the RNCC (RN-CM) SW-CM or SW.
9. Assist licensed staff with facility discharges as needed by updating/modifying plans upon re1evaluation and as delegated by licensed staff.
10. Help patients complete necessary paperwork and applications for services.
11. Communicate and coordinate post-discharge needs with community organizations and agencies for
resource referrals.
12. Facilitate follow-ups and check-ins with patients and other disciplines to ensure service and patient
satisfaction.
13. Performs other duties as assigned.
Bachelors degree in social work psychology sociology or other healthcare related field. No experience required 1-2 years preferred experience in healthcare setting (familiarity with healthcare or social service systems) such as in administrative or support role; Knowledge of community resources and health services preferred.
