General Position Summary:

Coordinates day to day activities and staffing for participants within the Day Break programs; encourages and supports participants to maintain their independence and daily living skills. Acts as liaison to appropriate agencies (i.e. County brokerages) residential providers and families to coordinate case management services. Assists Manager to develop and maintain systems which ensure the programs are successful premier providers of services. Works with program participants on a daily basis. Provides supervision to assigned staff. Is a positive and professional role model for others.

Essential Functions/Major Responsibilities:

  • Represents the programs and Garten in a positive and professional manner at all times.
  • Responsible for ensuring services provided are in compliance with OARs.
  • Maintains contacts with agencies involved (i.e County brokerages) residential providers and families.
  • Provides Case Management duties training including but not limited to intake assessment and exit process
  • Responsible for planning implementation and daily documentation of activities for participants
  • Completes required forms and all related paperwork (including but not limited to participant plans required billings etc.) accurately and in a timely manner.
  • Ensures ISPs are completed for all participants in a timely manner and tracks goal progress on a monthly basis
  • Ensures that medically related emotional and social needs of participants are met and maintained on an individual basis in accordance with policies and procedures and current federal state and local regulations and guidelines.
  • Responds to inquiries about program services in a positive and professional manner
  • Responsible for assisting in development and maintenance of policies and procedures to align with OARs and CARF Standards.
  • Ensures planned recreational activities occur and assists as assigned
  • Ensures appropriate transportation for outside activities
  • Ensures accurate and timely timekeeping and payroll documentation for assigned staff.

Ensures a safe and clean work environment exists which meet all safety requirements

Secondary Functions:

  • May represent Garten at meetings in the community (i.e. DD Services and other community partners).
  • Provides assistance and personal care to program participants in use of restroom and in clean-up as required.
  • Oversees petty cash management for program activities.
  • Participates in development of operating budget for division. Monitors and may approve expenses as assigned.
  • Participates in meetings and on teams/committees as assigned.
  • Provides input on ways the organization can create employment opportunities for people with disabilities.
  • Performs other related duties as assigned.

Specific Job Skills Required:

  • Excellent interpersonal verbal and written communication skills.
  • Excellent customer service

  • Ability to organize balance multiple priorities coordinate projects and prioritize work.
  • Ability to maintain a high level of confidentiality.
  • Ability to coordinate and supervise planned activities.
  • Ability to read write speak and understand English.
  • Ability to actively participate in team development and team-oriented processes.
  • Valid drivers license acceptable driving record and proof of current vehicle insurance.
  • Mental and physical ability to use a calculator and computer hardware/software (including word processor spreadsheets graphics and database programs).
  • Ability to maintain CPR/First Aid and OIS Certification.
  • Physical ability to push/pull wheelchairs and assist in lifting individuals out of wheelchairs.
  • Physical ability to carry up to 30 pounds frequently in a normal workday.
  • Physical ability to push/pull up to 200 pounds and to lift up to 100 pounds with assistance occasionally in a normal workday.

Preferred Skill:

  • Able to speak conversational Spanish.
  • Know American Sign Language.

Education and/or Experience Required:

  • An Associate Degree/equivalent training specializing in human services or related field
  • A minimum of 3 years experience working with seniors and/or individuals with disabilities.
  • A minimum of 1 year experience supervising a team of at least 4 professional staff
  • A 4-year degree in a related field may substitute for up to 2 years of experience.
