Job Description
Overview
The Case Manager at St. Josephs Recovery provides comprehensive case management and care coordination to support clients in recovery from substance use and co-occurring challenges. This position ensures access to essential services including housing healthcare public benefits and vocational resources while promoting self-sufficiency and family stability through wraparound support.

Responsibilities
- Coordinate individualized wraparound services for clients and their families.
- Assist clients with applications for Medicaid SNAP TANF housing vouchers and other public benefits.
- Support clients in accessing transportation child care primary care and behavioral health services.
- Develop and maintain resource guides and referral systems for internal and community-based supports.
- Collaborate with counselors peer support staff and external partners to ensure the delivery of integrated care.
- Monitor progress toward service plan goals and track referrals and outcomes in documentation systems.
- Advocate for clients needs in multidisciplinary team meetings and case conferences.
- Conduct outreach and follow-up to enhance retention and reduce service gaps.
- Maintain accurate timely records and ensure compliance with all applicable policies and funding requirements.

Qualifications
- Credential: Bachelors degree in Social Work Human Services Psychology or a related field.
- Experience: Minimum of 1 year of experience in case management care coordination or social services.
- Knowledge: Familiarity with public benefit systems housing programs and community resources.
- Skills: Strong organizational communication problem-solving and advocacy skills.
- Technological proficiency with electronic health records and basic office software.
- Preferred: Bilingual in English/Spanish; experience working with underserved or recovery populations.
Work Environment and Physical Demands:
- Office and community-based service settings.
Occasional local travel required for home visits outreach or accompanying clients.
Flexible schedule may include some evening hours to meet client needs.
