Salary: $127150.00 - $142254.44 super.

About the role

  • Managing the Chronic and Complex Care Service including heart failure endocrine/diabetes pain service co-ordinated care outpatients and COPD Services.

  • Actively contribute to the overall strategy and leadership within the clinical program providing sound financial administrative operational and human resource management for all clinical services within the portfolio.

  • Direction co-ordination and supervision of staff within the Chronic and Complex Care team to ensure safe and efficient clinical practice.

  • Proven ability to coordinate multidisciplinary teams ensuring high-quality patient care across a range of complex conditions and health care settings.

  • Strong critical thinking decision-making and problem-solving skills with the capacity to manage and prioritise.

About you

  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) or an Allied Health professional with relevant post graduate qualification in management and/or equivalent experience.

  • Demonstrated high level leadership within the workplace; ability to plan implement and evaluate local service delivery and to form productive working relationships.

  • Highly developed communication negotiation facilitation and conflict resolution skills.

  • Demonstrated experience managing operational financial human resource & workplace health and safety considerations.

  • Understanding of the role and function of community health services with ability to comprehend the navigation of a patients journey across hospital community and primary care services.
