The University of Miami Health System UHealth Department of Pediatrics has an exciting opportunity for a Full-Time Social Worker to work in Miami.
The Social Worker provides psychosocial assessments intervention treatment and discharge planning to patients and families to assist them in coping with their hospitalization illness diagnosis or life situations. The incumbent strives to meet the continuing needs of patients post hospitalization.
Core Qualifications:
Interviews clients and their families and coordinates programs and activities to meet their social and emotional needs.
Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle.
Provides psychosocial assessments of patients and families to identify emotional social and environmental problems.
Formulates and implements plans utilizing appropriate social work interventions.
Collaborates with the healthcare team and involves the patients and families in the development and implementation of plans.
Assists with the screening identification and management of victims of abuse neglect domestic violence and rape.
Makes referrals and serves as a resource for the healthcare team and serves as a patient advocate.
Maintains a working knowledge of relevant medical and legal issues and preserves records and statistics.
Attends and participates in staff committee department and other administrative meetings.
Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions:
Conduct assessments and provide direct services to students and families identifying needs and creating individualized service plans. Ensure that students receive the appropriate health behavioral and social interventions.
Work closely with school personnel including teachers school nurses administrators and counselors to ensure integrated and coordinated care for students. Act as a liaison between families and community health services.
Advocate for students health and well-being helping to navigate and access resources such as mental health services healthcare coverage housing assistance and financial support.
Provide short-term crisis intervention and support for students and families facing immediate health behavioral or social challenges.
Maintain detailed and confidential case records ensuring compliance with HIPAA and school district policies. Regularly track and report progress outcomes and trends.
Establish and maintain relationships with local community agencies healthcare providers and public service organizations to enhance service delivery and ensure resources are effectively utilized.
Work effectively with diverse populations respecting cultural values and addressing unique barriers faced by students from various backgrounds.
Other job-related duties as assigned.

CORE QUALIFICATIONS:
Bachelors degree in relevant field required
Minimum 1 year of relevant experience required
Learning Agility: Ability to learn new procedures technologies and protocols and adapt to changing priorities and work demands.
Teamwork: Ability to work collaboratively with others and contribute to a team environment.
Technical Proficiency: Skilled in using office software technology and relevant computer applications.
Communication: Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.
Any appropriate combination of relevant education experience and/or certifications may be considered.
