About the role:
The Social Work Team works alongside patients families and carers who need palliative social work support. The role requires working with patients in the inpatient unit and community settings including patients homes.
The team works collaboratively with families and carers community nursing teams wider hospice services and external organisations to support people in the advanced stages of illness to access the support they need and plan for the future.
The successful candidate will join a friendly and professional team who make areal difference to patients and their families at the most challenging time. This role allows the time to build trusting relationships and the opportunity to support in creative ways.
About the person:
You will manage your own caseload therefore the ability to work independently is essential.
We are looking for someone who can support patients families and carers with sensitivity and compassion.
You will have experience of working with people approaching the end of their life in a health or social care service
You must have a clean driving licence and access to a car as we work across Bristol South Gloucestershire and North Somerset.

Due to the nature of the work involved this role is exempt from the ROA and thejobholderwill berequired to undergo a Disclosure and Barring Service Check.
We will also require a vaccination history or evidence of immunity.
We will be screening and offering interviews as we receive applications and therefore strongly encourage applications as early as possible to avoid disappointment.
