The Human Trafficking Coordinator (HTC) will be responsible for coordinating patient care plans by liaising with all necessary care providers and medical professions to ensure patient needs are met coordinating follow-up appointments and referrals for patients to specialists and community-based services as needed and coordinating internal Human Trafficking Hospital Program monthly meetings.
Principal Duties:
1. Meet with patients and patient families to ensure their needs are being met.
2. Do daily rounds to the Emergency Department Behavioral Health OB/GYN and Ambulatory units to review patient charts and coordinate with healthcare providers to flag indicators of trafficking conduct in- depth relationship-based patient assessments for sex and/or labor trafficking.
3. Coordinates patient care plans by liaising with all necessary care providers and medical professionals to ensure patients needs are met.
4. Ascertains patients post-discharge needs coordinate patients follow up appointments and connect patients to appropriate specialists and community-based providers.
5. Coordinate internal Human Trafficking Hospital Program monthly meetings develop meeting minutes and follow-up on next steps and outstanding logistical programmatic steps.
6. Work with the Human Trafficking Hospital team to revise screening identification treatment and referral workflow in 5 units of the hospital.
7. Attend target departmental meetings and stay up to date on all business initiatives related to the Human Trafficking Hospital Program.
8. Collect and input patient data insurance information and financial information into computer system.
9. Responds to needs of all crisis line callers in an appropriate manner in accordance with written procedures.
10. Accurately assess lethality and initiate appropriate intervention with suicidal callers.
11. Provides crisis callers with accurate information and make appropriate referrals.
12. Appropriately triage patients on arrival and prioritize and process cases in an efficient courteous manner.
13. Accurately identify and differentiate immediate crisis and underlying pathology.
14. Assess the problems and provide emotional support to patients in crisis while setting limits.
15. Conduct empathetic objective interviews with psychotic patients. Approach depressed and/or suicidal patients in a non-threatening and supportive manner demonstrating empathy while maintaining a therapeutic relationship.
16. Obtain pertinent information from involved individuals while assessing a patient who is on an ECP; provide accurate information to individuals who have filed an ECP.
17. Assess the potential for violent behavior and respond appropriately to assure the safety of patient and staff; de-escalate volatile situations according to written procedures.
18. Assist in the management of stretcher patients including changing sheets taking specimens to lab obtaining lab results and documenting observations of restrained patients.
19. Collaborate with physicians and nursing staff in all aspects of patient care and appropriately consult with physician regarding case management.
20. Provide clear follow-up instructions to patients and their families to assure compliance with treatment plan.
21. Document demographic and clinical information in the clinical record and complete all paperwork related to case management in accordance with quality assurance standards and written procedures. Write concise coherent and accurate clinical summaries.
22. Accurately identify and document patients chief complaint and precipitating events.
23. Document clear psychosocial history of each patient.
24. Obtain collateral information from families care providers and significant others when necessary.
25. Make appropriate disposition plans with psychiatrist and other agencies to assure continuity of care.
26. Obtain pertinent clinical information from substance abuse patients assess motivation for treatment and potential for withdrawal reaction. Recommend treatment plan based on assessment.
27. Correctly identify types of visits and catchment areas and completes central log Kardex and billing slips. Accurately complete paperwork necessary for admissions voluntary and involuntary transfers ECP reimbursement registration certification and billing.

1. Greets customers in courteous friendly respectful and professional manner at all times including maintaining eye contact when appropriate.
2. Follows communication protocols to both internal and external customers including introducing him/herself with job title and experience asking open ended questions such as How may I be of help to you using the customers name as soon as it is learned.
3. Responds promptly and appropriately to customer questions/concerns/complaints and attempts immediate resolution.
4. Keeps customers information confidential including public places such as elevators or the cafeteria.
5. Provides assistance and offers help immediately including finding someone else to meet the request if unable to do so him/herself. Introduce other staff to customers when a hand-off occurs and explain that the person will provide excellent service.
6. Demonstrates commitment to excellent service recovery when a customers expectations have not been met.

Communication Etiquette:

2. Refrains from use of personal cell phone in patient care areas and keeps usage to a minimum at all other times while on duty.
3. Does not text or use e-mail during meetings (except for exigent or emergency situations).
4. Limits use of business cell phone during meetings (remain on vibrate and/or calls go to voicemail).
5. Makes every effort to answer telephone calls within three rings introducing him/herself department and title (if appropriate). Asks permission before placing the caller on hold or using the speakerphone. If caller is transferred gives the caller the extension number of the person he or she is being transferred to. Offers further assistance to the caller upon completing the conversation.
6. Maintains an appropriate voicemail message and when away from the office has an out-of-office email message that is brief current and includes name and department and offers the caller options if possible.
7. Returns email and voicemail messages promptly but no later than within one business day (24 hours).
8. Always mindful of voice and language in public.
Self Management:
1. Reports to work appropriately groomed and in compliance with the Hospitals dress code. Wears identification badge at all times at chest level and facing outwards so identification is clearly visible.
2. Complete all assignments within deadlines or negotiates alternative actions and time frames in order to achieve desired outcomes.
3. Completes mandatory annual education and competency requirements.
4. Follows UMCAP safety infection control and employee health standards.
5. Demonstrates responsibility for personal growth development and professional knowledge and competency.
6. Adhere to all UMCAP and department policies and procedures including Code of Conduct and professional behavior standards. Does not exceed Hospital guidelines in reference to attendance punctuality and use of sick and unplanned absences. Provides notification of absences lateness and vacation requests according to department guidelines. Respects length of time for lunch and break times.

Qualifications :
POSITION REQUIREMENTS:
Licensure/Certification/Registration
Required: There are no licensure requirements for this position
Life Support Certification:
Basic Life Support Health Care Provider (BLS-HCP)
Education/Knowledge
Attained Level: Professional. A Bachelors degree OR any equivalent combination of training and experience that provides the required knowledge skills and abilities.
Preferred: Masters degree in social work Human Services Nursing or a related field. Courses in human trafficking and trauma-informed care.
Completed Course Work/Program: Bachelors degree from an accredited college or university in Social Work Human Services Nursing or related field OR any equivalent combination of training and experience that provides the required knowledge skills and abilities.
Applicable Experience
Required Experience (years): Three
Preferred: 3-5 years
Experience (describe required & preferred): Minimum 3 years working in a clinical setting background in trauma informed care human trafficking and or domestic violence/sexual assault 1 year of proven success in leadership roles superior interpersonal communication skills and the ability to exercise empathy when working with patients and their families strong organizational and time management skills a demonstrated an understanding of standard clinical procedures laws and regulations and some knowledge of medical terminology. Ability to speak a second language is an asset.
