Job Description
Location: 1500 Forest Glen Rd. Silver Spring MD 20910
Shift: 8:00 AM 4:30 PM (M F EOW Required)
Pay Rate: $55/hr (W2) $60/hr (1099)
- License: Active Maryland LCSW required to interview
- Experience: Recent experience in acute care discharge planning (hospital setting)
- Collaborate with interdisciplinary teams to facilitate timely and effective discharge plans
- Perform comprehensive psychosocial assessments
- Provide counseling and support to patients and families
- Coordinate community and post-acute resources
- Advocate for patients needs and ensure continuity of care
- Document care plans interventions and outcomes accurately and efficiently
