Coordinated Entry Specialist Covers Multiple Counties
Supportive Services for Veteran Families (SSVF)
Full-Time 40 Hours/ Week
$42000/Year
GENERAL STATEMENT OF DUTIES: Establishes and strengthens coordinated entry processes for homeless veterans across programs geographical service areas using SSVF as an access point.
ESSENTIAL RESPONSIBILITIES:
1. Establishes working relationship and collaborates with VAs Coordinated Entry Specialist
2. Supervises and trains outreach staff on coordinated entry diversion and triaging practices.
3. Oversees and evaluates Family & Community Services veteran support line practices.
4. Participates in CoC meetings and planning efforts which may include leadership roles within the CoC group.
5. Provides direct linkage from community services to VHA homeless program services through regular and consistent outreach and communication with community providers.
6. Participates in case conferencing process within the CoCs coordinated entry system to help inform and facilitate referrals to VA programs such as HUD-VASH SSVF and GPD.
7. Ensures efficient sharing of Veteran data and program information as allowable under HIPPA. This will include knowledge of HMIS data standards data sharing and privacy authorities HUD guidance and other applicable documents or established guidance.
8. Collects and provides required data for the HMIS database as applicable. Develops partnerships with local VA programs with regular contact and communication.
9. Evaluates the effectiveness of resources and referrals provided and makes appropriate modifications to ensure the provision of high-quality care and interventions.
10. Assists in development of policy and procedures using knowledge gained from research or best practices.
11. Actively participates in regular SSVF Team meetings and other coordination meetings for the purpose of consistency monitoring and effectiveness.
12. Applies Motivational Interviewing best practice models in service delivery approach.
13. Provides crises intervention when necessary.
14. Participates in community-wide events promoting services for veterans.
15. Complies with agency policies and procedures COA regulations federal and state requirements and educational/certification/registry requirements.
16. Other duties as assigned.
QUALIFICATIONS:
1. Demonstrated organizational interpersonal communication and systematic evaluation skills.
2. Possesses excellent judgement
3. Valid Ohio Drivers License and ability to meet agency requirements for driving insurability if applicable; reliable transportation
4. Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation).
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Masters level social worker or equivalent education is preferred.
MINIMUM EXPERIENCE REQUIREMENTS: Five years working in social service area of practice. Experience with VA/HUD housing and homelessness projects.
