About the Role:
Fixed-term full-time position playing a pivotal role within our Alcohol and Drug Service delivering expert social work support to patients families and carers throughout their care journey. Youll lead comprehensive psychosocial assessments expertly manage risk and develop personalised treatment plans that address complex alcohol and drug challenges alongside broader health and social needs.
Coordinate safe effective discharge planning ensuring seamless referrals to community supports and facilitating access to appropriate short- and long-term accommodation or placement options that best support patient recovery.
Provide compassionate individual and family counselling youll help patients and their loved ones navigate the emotional complexities of medical conditions lifestyle adjustments and experiences of grief and loss fostering resilience and hope.
As a strong advocate you will manage complex applications and referrals to key agencies such as NCAT Guardianship Division NDIS Housing NSW Department of Home Affairs My Aged Care LTCS and CTP claimsensuring patients and families receive holistic care and vital resources.
Beyond direct client work you will lead and contribute to multidisciplinary team activities participate in clinical governance and quality improvement initiatives and support the professional development of social work staff and students shaping the future of psychosocial care within the service.
About You:
You bring a Bachelor of Social Work and eligibility for AASW membership backed by solid clinical experience in tertiary health settings.
Youre confident navigating challenging areas like alcohol and drug interventions harm minimisation and co-morbid health issues. You also handle sensitive situations involving domestic violence and child protection with empathy skill and a trauma-informed approach.
You thrive working alongside vulnerable clients and families facing complex health and psychosocial challenges. Your holistic compassionate approach helps empower individuals in community and hospital settings to achieve better outcomes.
Experienced social work leader. You guide multidisciplinary teams with confidence provide meaningful clinical supervision and education and foster a positive collaborative environment where professionals grow and thrive.
Excellent communication skills combined with your ability to lead clinical planning research and quality improvement projects make you a trusted consultant and collaborator.
