Licensed Clinical Social Worker Palliative Care & Hospice
Full-Time or Per Diem $45$50/hr

Senior Doc Palliative Care is hiring a Licensed Clinical Social Worker (LCSW) to join our growing mission-driven team. Founded by Dr. Geiss in 2014 Senior Doc has become a leader in personalized patient-centered care. We specialize in supporting seniors especially those with dementia through interdisciplinary care that integrates pharmaceutical expertise to improve quality of life.

What Youll Do:
Psychosocial Support: Assess the emotional social and psychological needs of patients and caregivers
Care Coordination: Collaborate with physicians nurses and care teams to build effective care plans
Family Education & Advocacy: Help families access resources and navigate complex care decisions
Bereavement Services: Provide post-loss counseling and support to families for up to a year
Regulatory Compliance: Maintain accurate and timely documentation in accordance with state/federal standards
What Were Looking For:
License & Credentials: Active LCSW license required; NPI and credentialed to bill for CBT
Experience: Preferred experience in palliative care hospice geriatrics or chronic disease management (including home health)
Skills: Compassionate autonomous team-oriented and excellent at time management and communication
Other Requirements: Valid drivers license and insured vehicle for patient visits as needed
