Job Description:
At Elara Caring we believe the best care is delivered at homewhere patients feel most comfortable and supported. As a PRN Spiritual Coordinator youll play a vital role in honoring that belief by offering emotional and spiritual support during one of lifes most profound journeys.
This is a flexible PRN opportunity ideal for someone who is called to serve and available to provide coverage during planned staff PTO vacations or peak care demands. Youll join a compassionate interdisciplinary team dedicated to providing holistic patient-centered hospice care.

You will offer guidance presence and compassion to patients and families across diverse spiritual and cultural backgrounds. Key responsibilities include:
Conducting initial and follow-up spiritual assessments to identify patient and family needs.
Developing individualized spiritual care plans as part of the interdisciplinary team.
Providing non-denominational spiritual counseling prayer support and guidance on topics such as meaning forgiveness faith and grief.
Participating in team meetings to ensure spiritual care is fully integrated into the care plan.
Facilitating discussions that bring peace and closure to patients and their loved ones.
Offering pre- and post-bereavement support in alignment with hospice philosophy.
Educating patients families and staff on the role and value of spiritual care.
Documenting visits and interventions in compliance with agency state and federal requirements.
Respectfully honoring and advocating for the beliefs values and cultural practices of those you serve.
Bachelors or Masters degree in Divinity Theology Pastoral Care or related field.
Ordination or certification as a chaplain clergy member or spiritual counselor.
Minimum 2 years of experience in hospice ministry chaplaincy or counseling setting.
Ability to travel throughout the service area (approx. 50% travel).
Strong interpersonal and communication skills.
Valid drivers license active auto insurance and reliable transportation.
Physical ability to sit stand bend and lift up to 50 lbs as needed.
