TheHigh-Risk Patient Engagement Specialistfor Village Medical at Home is responsible for providing patient support and communication while facilitating an exceptional patient experience to build and reinforce satisfaction and trust and drive organizational loyalty.
How You Will Get Things Done:
Answers the high-risk programs phone line and connects patients to the clinical care team for medical needs
Outreaches to high-risk patients to enroll in the high-risk programs
Outreaches to eligible referrals intakes demographics and insurance information and schedules new patient visit series
Monitors inboxes for new patient referrals and confirms program eligibility
How You Will Build Trust:
Communicates outcome of new patient outreach to the PCP or other referral source
Monitors online check-in pushing reminders for online check-in completion and remotely checking in patients as needed
How You Will Innovate:
Schedule new patient appointments/follow-up appointments per established policies and procedures
Experience to Drive Change:
Current BLS certification for healthcare providers preferred; required within 90 days of employment
High School Diploma/Equivalency
2 years of experience in primary care family practice internal medicine urgent care or ER preferred
Valid drivers license
