Responsibilities
Pharmacist St. Bernardine Medical Center
Join a mission-driven healthcare team that prioritizes patient safety professional collaboration and compassionate service. St. Bernardine Medical Center is seeking a skilled and detail-oriented Pharmacist to provide comprehensive pharmaceutical care across inpatient services. In this vital role you will ensure safe medication practices participate in performance improvement initiatives and serve as a trusted resource for clinical staff and providers.
Key Responsibilities:
- Review process and dispense medications and IV therapies with precision and in compliance with all hospital policies and regulatory guidelines
- Provide direct clinical consultation and medication therapy recommendations to physicians and nursing teams
- Supervise and support pharmacy technicians in compounding repackaging and medication distribution
- Monitor patient medication profiles for drug interactions allergies and contraindications
- Perform pharmacokinetic calculations and adjust dosages as appropriate for patient age weight and condition
- Support formulary compliance therapeutic interchange and clinical pharmacy initiatives
- Participate in department performance improvement projects medication use evaluations and adverse drug reaction monitoring
- Ensure accurate and timely documentation within the EMR and pharmacy systems
- Maintain compliance with controlled substance protocols regulatory standards and documentation requirements

Qualifications
- 1-2 years experience in Hospital Pharmacy or completion of a Pharmacy Residency program preferred
- Experience in unit-dosing IV admixtures pharmacokinetics and TPN desire
- Graduate of a School of Pharmacy required Pharm D. desirable
- Basic knowledge of computers desirable
- California Pharmacist License
