What do you offer

The Second Stage Housing Case Manager will work as part of the Housing Services Team that provides programs and services for individuals with affordable/social housing needs. The Case Manager will work with the Director to achieve strategic goals in the fulfillment of the mission of the YMCA of the National Capital Region.

Some of the tasks of this position include:

  • Providing general information to individuals and agencies regarding eligibility for housing in the Ys Housing Services and specific information about the Second Stage Housing Program.
  • Scheduling and conducting interviews with housing applicants and their supports to assess eligibility and compatibility for the Second Stage Housing Program.
  • Developing and maintaining individual goal plans with assigned clients.
  • Providing case management counselling support crisis intervention and referrals for clients.
  • Providing life and social skills workshops.
  • Maintaining standards for reporting and operating procedures.
  • Reporting statistical data.
  • Ensuring client confidentiality and that all files and records are secure.
  • Building and maintaining relationships with relevant agencies and services to promote referrals to the Second Stage Housing Program.
  • Maintaining the workplace to ensure a safe and clean environment.

What qualifications and abilities are we looking for

The successful candidate will have the following:

Education

  • Post-secondary education or equivalent in a related field

Language

  • Fluent in English and French oral and written
  • Other languages are an asset

Experience Skills and Abilities

  • Minimum 2 years of experience in social services preferably with youth
  • Minimum 1 year of experience in the housing field preferred
  • Knowledge and sensitivity in dealing with individuals with homelessness addiction and mental health
  • Knowledge of community resources relevant to youth
  • Training in suicide intervention and non-violent crisis intervention preferred
  • Proficiency with Microsoft Office Suite
  • Proven facilitation skills for workshops and information sessions

Additional Requirements:

  • Applicants must have a current Standard First Aid and CPR C certification.
  • Applicants must already have a valid permit to work in Canada.
  • A Police Record check not more than 6 months old is required. Police Records checks are reviewed on an individual basis and the offense(s) - if any - is considered in the decision-making process in relation to the requirement of working with children therefore not eliminating all candidates with a record from being offered a position.
  • Semi-annually staff must complete the mandatory training for the protection of Children & Youth and submit the certificate of completion.

  • Quality Focus and Results Oriented: Provides high standard of service delivery consistent with contractual obligations.
