Job Description

TEMPORARY UNTIL APPROXIMATELY JUNE 2026


Reporting to the Manager Housing Facilities & Employment Services the incumbent working within a multidisciplinary team is responsible for assertive outreach to people in non-traditional settings. The position will work with individuals to gain rapport educate and establish community linkages and treatment plans.

RESPONSIBILITIES & DUTIES:

The position is responsible for the following:

  • Assertive outreach to individuals in non-traditional settings.
  • Establishing rapport and forming trusting relationships that provide the opportunity to assess needs and gain mutual agreement with the client on a treatment plan.
  • Working with a multi-agency team at point of care improving comprehensive case planning by assessing needs and gaining agreement on a plan of care.
  • Improving access to care by conducting comprehensive case planning promoting linkages to appropriate resources and by providing education and assistance interfacing with the health care and social service systems.

EDUCATION & SKILLS:

  • Knowledge and skill acquired through the completion of a three (3) year University degree in a field deemed relevant by the employer (i.e. degree in Criminology Psychology Law or other related field) or a minimum of a two year post-secondary diploma in a relevant discipline accepted by the employer such as Developmental Service Worker.
  • One (1) year recent relevant experience in the judgement of the employer working with and teaching life skills to vulnerable individuals particularly those with a mental illness or related diagnosis.
  • Comprehensive knowledge of mental illnesses signs and symptoms as well as planning and organizational skills. Strong assessment skills. Knowledge of community resources.
  • Computer literacy skills and working knowledge or various Microsoft Windows application is required.
  • Bilingualism in both official languages at the advanced level is considered an asset.
  • A valid Ontario Drivers License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.

Hours of Work:
Twenty (20) hour work week. The agencys regular hours of operation are Monday to Friday 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.

Salary range: Grade 8 $35.71 $42.25 per hour

Per 2022 - 2026 Collective Agreement

Applicable shift premiums are paid in accordance with the Collective Agreement Article 17.11.
