1-2 Month Contract with a local Authority
Job Purpose
- To provide compassionate person-centred support to vulnerable adults in the community including those experiencing homelessness mental health challenges substance misuse or social isolation. The Outreach Support Worker will engage individuals in their own environments build trust and empower them to access essential services improve their wellbeing and work towards greater independence and stability.
Key Duties and Responsibilities
- Conduct proactive outreach to identify and engage individuals sleeping rough or in unstable housing situations.
- Build and maintain supportive non-judgmental relationships with clients to encourage trust and engagement.
- Carry out needs and risk assessments to develop personalised support plans.
- Support clients in accessing emergency accommodation longer-term housing and tenancy sustainment services.
- Assist with benefit applications including Universal Credit Housing Benefit and PIP and liaise with relevant agencies to resolve issues.
- Accompany clients to appointments (e.g. health housing legal) and advocate on their behalf where necessary.
- Work closely with multi-agency partners including local authorities health services police and voluntary organisations.
- Maintain accurate and timely records of all client interactions using case management systems.
- Participate in team meetings supervision and training to ensure continuous professional development.
- Respond appropriately to safeguarding concerns and follow organisational policies and procedures.
- Contribute to service development by sharing insights feedback and best practices.
Essential Experience Required
- Experience working with vulnerable individuals particularly those affected by homelessness substance misuse mental health issues or offending behaviour.
- Strong interpersonal and communication skills with the ability to build rapport and maintain professional boundaries.
- Knowledge of housing systems welfare benefits and support services.
- Ability to work independently and manage a caseload effectively.
Essential Qualifications Required
- Level 3 qualification e.g. BTEC Environmental Health or 2 A levels NVQ3 or equivalent in relevant subject(s)
