Job Preview at a Glance:
The Case Manager serves as a central support within the interdisciplinary team working primarily with adults. This staff member conducts assessments assists members with resources within the community and provides coordination of care inside and outside of the agency.
Pay Rate: Starts $17.50 hourly
Responsibilities:
Intake screenings (Completing intake paperwork and referrals)
Completing annual behavioral health assessments including service planning and support planning
Facilitating CFTs and ARTs
Facilitating groups
Identifying gaps in care for our members
Completing SMI and SED evaluation
