Your primary role is to work collaboratively with your team to provide direct rehabilitation care in speech therapy to residents upon physician referral as well as to adequately plan for resident needs.
- Meet the residents goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining speech therapy treatment plans in consultation with physicians or by prescription
- Administer speech therapy treatments
- Complete discharge planning by consulting with physicians nurses social workers and other health care workers; contributing to resident care conferences
- Assure continuation of therapeutic plan following discharge by instructing residents families and caregivers in home care programs; recommending and/or providing assistive equipment; recommending home health or follow-up programs
What Were Looking For:
You will be able to demonstrate composure customer focus patience and the ability to gain trust and establish rapport with residents and your team.
- A strong desire and passion to work in an environment with people that are unwell disabled or living with dementia. Prior experience with mental health and psych population is preferred.
- Must be a graduate of an accredited program in speech therapy (minimum MSSLP)
- Must be ASHA certified (CCCs) or must be eligible for 9-month Clinical Fellowship Year (CFY) in order to earn ASHA certification (CCCs) and/or meet ASHA credentialing criteria
- One years experience in long-term care or related setting preferred
