Shift: Days (830-5p) no weekends On Call Requirements: N/A Holiday requirements: 2 major and 2 minor Weekend requirements: No Special skills and/or minimum experience ideal candidate should possess Acute hospital case management x 2 years ***Candidate should posses RN license or LMSW minimum licensure Lic Clinical Social Worker GA 1.00 Required Lic Master Social Worker GA 1.00 Required Basic Life Support 2.00 Required BLS - Instructor 2.00 Required BLS - Provisional 2.00 Required Certified Case Manager 3.00 Preferred Accredited Case Manager 3.00 Preferred 2 years of experience in healthcare in the acute care setting related field or skilled care environment or community. Required and A background in medical social work in an acute care setting Preferred
