Overview:
We are seeking a Board Certified/Board Eligible Family Medicine Physician to join a well-established patient-centered outpatient practice in Seymour Connecticut. This full-time permanent role offers an excellent work-life balance collaborative environment .

Key Responsibilities:
Provide comprehensive outpatient primary care for patients of all ages
Conduct wellness visits chronic disease management and urgent care as needed
Collaborate with nurses specialists and support staff to coordinate care
Maintain accurate documentation in a fully integrated EMR system
Participate in quality assurance programs and ongoing professional development
Requirements:
MD or DO degree
Board Certified or Board Eligible in Family Medicine
Active or eligible Connecticut medical license
Strong commitment to patient-centered care and community health
Excellent communication collaboration and problem-solving skills
