Job Description
Required Tasks
- Proactively identify and connect with at-risk youth in the community.
- Provide guidance support and resources to youth involved in the legal system including those with out-of-custody warrants.
- Conduct needs assessments to determine appropriate interventions and services.
- Develop individualized action plans in collaboration with youth and relevant stakeholders.
- Monitor youth progress and provide ongoing support and advocacy.
- Establish and maintain strong relationships with schools acting as a resource and liaison for at-risk youth.
- Collaborate with community partners such as social service agencies mental health providers and law enforcement to create a network of support for youth.
- Participate in community events and initiatives focused on youth development and well-being.
- Maintain accurate and up-to-date records of youth interactions and services provided.
- Enter data into the JAIS computer system for tracking and reporting purposes.
- Develop and continuously update a resource booklet with comprehensive information on available services for at-risk youth.
- Assist in the development and implementation of new programs and initiatives to address emerging needs of at-risk youth.
- Perform other related tasks as assigned.
Education and Experience Requirements
- Associates Degree in Social Work or other related human services area.
- Minimum of one (1) year of experience in child welfare juvenile justice mental health or other setting providing services to youth and families.
Special Conditions
- Candidates must receive Child Welfare Licensing/Child Protective Services clearance.
